Two Important Items

USPS Postal Consent Form and Change of Address Form 1375

USPS Form 1583

USPS regulations require you to complete a form 1583 postal consent form for each recipient to authorize us to handle your mail. Without a completed USPS Form 1583 we cannot process your mail and will have to return any mail received to senders.

Form 1583 must be signed in front of us or notarized. A quick Google search will find notaries near you. Also, online notaries are available. We recommend Notarycam.com

Two forms of ID are required. Below is a list of acceptable IDs.

ID

  • Valid driver’s license or state non-driver’s identification card
  • Passport, alien registration card, or certificate of naturalization
  • Armed Forces, Government, University, or recognized Corporate ID
  • Voter or vehicle registration card
  • Current lease, mortgage, or deed of trust
  • Insurance policy (home, health, or vehicle)
  • Matricula Consular (Mexico): A Matricula Consular card is an identification card issued by the Government of Mexico through its consulate offices to Mexican nationals residing outside Mexico.
  • NEXUS (Canada): NEXUS is a joint Canada Border Services Agency and U.S. Customs and Border Protection operated trusted traveler and expedited border control program.  To be accepted, the NEXUS card must contain an identification number.

Unfortunately, social security cards, credit cards, and birth certificates are NOT acceptable forms of identification.
